After Stroke BC is a personalized stroke recovery program – delivered in affiliation with March of Dimes Canada – that helps survivors, and their families navigate the path forward after a stroke. There are about 6,800 strokes in BC every year, and the need for our services is growing. One in four affected by stroke is under 65 years of age.
The transition from hospital to home can be a particularly challenging time for stroke survivors and their families as they leave the supportive environment of that setting. The gap in existing infrastructure of stroke support is a pressing issue in BC for the stroke survivors who are discharged each year and are faced with trying to navigate the systems of support on their own. This systemic gap results in increased re-hospitalizations, secondary strokes, and leaves stroke survivors and their caregivers feeling isolated and alone.
Our After Stroke Coordinators are here to help bridge that gap.
The objectives of After Stroke BC are to:
Provide timely and relevant support, connection, and resources to stroke survivors when and where they need them most
Improve stroke survivors' confidence and ability to access and use community-based services; increase their use of these services
Increase stroke survivors' sense of social connectedness and empowerment and improve quality of life (e.g. improved mental health, ability to choose to live at home, reintegration into their community)
Decrease stroke survivors' use of primary and/or urgent health care
Our program is 100% donor funded and is free. These funds will cover the cost of additional After Stroke Coordinator hours to provide community navigation & case management services for stroke survivors and their families as they transition from hospital to home in their journey of recovery.
